* Residents, Fellows and Students: FREE with pre-registration
Cancellations
Cancellation requests must be received by October 5, 2012. No refunds will be made after that date.

Room reservation rates INCLUDE dinner on Friday, October 12, in the dining room from 5:30-8:00 pm (or boxed dinner for late arrival with prior notification), and overnight accommodations. The special discounted rates are as follows:
- Meeting attendee single occupancy is $199 + tax
- Meeting attendee double occupancy is $175 + tax per meeting attendee
